What would cause this?

I have a 1994 plymouth grand voyager that recently had the drivers side axle replaced 3 times all within 1 month the axle keeps going out and recently the power steering pulley has broken 2 times as well. I have replaced all of these parts with new parts not used but they keep going bad any ideas on what could be causing this?
